Annual Weapons Report to OSPI - Action Required

Dear School Leaders,

Before you leave your building for summer vacation or more, if you have not completed your weapons report for the 2013-14 school year, please do so as soon as you can. You do have until August 1, but returning to school for the new year can be a hectic time.

Go to https://eds.ospi.k12.wa.us; select Behaviors & Weapons from either View My Applications or My Applications (DO NOT USE ATTENDANCE & WEAPONS). When the page opens, click on the Search button to bring up the form. The default is set at “yes, we have weapons . . .”—if you do not, click on the “no” option and submit.

If you do have an incident(s) to report, move down to the body of the report and fill in the information. Submit when completed. You will see a green or gold box with the name of the person completing the form and date when a success submission has been received.
