A special request:
WA has a high medical exemption rate (6 times higher than the national average), which we believe may be related to the exemption law change in 2011. Please review Certificates of Exemption with a medical exemption checked, in addition to a personal exemption, and follow-up with parents to determine if there’s a reason for the medical exemption. Medical exemptions should be obtained only if there’s a medical contraindication to vaccination. Providers may be confused about filling out the form.
Parents can request immunization records or the required Certificate of Immunization Status from our state Immunization Information System. Please look at our web page for more information.

How to Report School Data
Report in the Immunization Information System: www.waiis.wa.gov.
- If you are new and will use the online Immunization Information System (IIS) to report, please send an email to waiishelpdesk@doh.wa.gov to request an account setup. Please remember: if you already have access to the IIS to look up students’ immunizations, you still need another account to report in the IIS.
- If you reported in the IIS last year, you will get an email with your username and password this week. If you don’t see this email, please send an email to oicpschools@doh.wa.gov.
How to Report PreSchool Data
This year, we are asking all preschools and child cares to fill out the preschool report form and email or mail the form to the Department of Health. There is no online reporting form this year.
Do NOT use the IIS to report preschool data.
- Send all questions related to reporting to: oicpschools@doh.wa.gov.
- All public and private schools, and licensed preschools must report the immunization status of students by Nov. 1 each year per state law (WAC 246-105-060).
