Immunization Status Reporting
· All private schools and licensed preschools must report the immunization status of students by Nov. 1 each year per state law (WAC 246-105-060).
· You can report in two ways:
o Use the WA State Immunization Information System (IIS): www.waiis.wa.gov or
o Email the electronic data file (excel or csv) for schools AND preschools to oicpschools@doh.wa.gov if you use a student information system such as Skyward or PowerSchool

· If you are new and did NOT use the system to report last year, please register to use the system: www.doh.wa.gov/CommunityandEnvironment/Schools/Immunization/SchoolStatusReporting.aspx
· Please remember: if you already have access to the IIS to look up students’ immunizations, you will need another username to report in the system
· Do NOT use the IIS to report preschool data. If you can’t email an electronic data file, report online here: http://survey.constantcontact.com/su...74cc/questions

Exemption Information

· We have heard some schools are mistakenly asking parents who request a personal exemption to get a healthcare provider signature on the Certificate of Exemption in the box for a medical exemption. Only exemptions that are truly for medical reasons require the provider signature in the Medical Exemption box.
