Dates and Procedures for 2014 - 2015 OSPI Forms and Applications

Dear School Leaders,

As we welcome new leaders into our 74 schools, it is important to make sure all schools know that there are 4 OPSI compliance reports that all 74 schools must complete annually based on Washington state laws.

We are grateful to work in partnership with Laura Moore,our Private Education liaison at OSPI. Laura is happy to help our schools secure updated information and access.

Please note the following timeframes to anticipate the OSPI bulletins/memos which will be emailed to you regarding when forms or applications are due from your school. If you or a staff member does not have an account in the EDS system (all applications are in EDS and completed electronically), please communicate with Laura Moore directly and she will create a new account for you. If an account is needed for a staff member, please send an email to Laura Moore as the head of the school giving access permission to your delegate.

1) Enrollment & Staff Reporting (P105B)
Bulletin emailed September 30. Application opens October 1/Closes November 17. THIS IS A CHANGE. Enrollment as of October 1; all instructional staff and head of school must be reported.

2) Private Participation in Federal Programs
Bulletin emailed in mid-January. Dates not set for opening/closing. This is the selection of services provided by local public school districts from federal funds. Even if you are opting out, the form must be completed.

3) Private Education Approval
Bulletin emailed February 19. Application opens February 20; applications need to be completed by April 14 for May approval/June 9 for July approval—THIS IS A CHANGE. This is the re-approval application for the 2015-16 school year.

4) Weapons Report
Bulletin emailed in mid-May . Date not set for opening/applications will be due mid-July. This is a report of any weapons incidents at your school during the 2014-15 school year. The form must be completed even if you have no incidents to report.
