State Required Reporting - OSPI

As we start the new school year, I wanted to provide you with a list of dates when reports will be due from each school. Several people asked why they had to do the Weapons report—it is required by state law (RCW 28A.230.130).

Enrollment and Staff Reporting (P105B) Memo will be mailed on September 30; reporting time—October 1 thru November 18

Private Education Approval (re-approval form) Memo will be mailed on February 18; completion time: February 19 thru April 12 for May Approval
April 13 thru June 14 for July Approval

Private Participation in Federal Program Memo will be mailed in mid-January: completion time: mid-January thru early March

Weapons Report Memo will be mailed early to mid-May: completion time: mid-May to July 15

If anything about your school has changed over the summer—administrator, email address(es), location of school, etc., please notify me immediately. I need to have the most up-to-date information to be able to contact each school. If you location has changed, I will need the new address, new health and fire safety inspection reports, and any other changes. The health and fire safety inspection report forms can be found at http://www.k12.wa.us/PrivateEd/Priva...plication.aspx near the bottom of the page.

If anyone needs an account established in EDS for reporting purposes, please let me know. If you need one for a new staff member, please send me an email authorizing access and the roles required along with the person’s name and email address.
