Please Register for a free Webinar on Sept 27 to learn more about the School Immunization Requirements and Reporting. The webinar will cover; immunization requirements, including some of the more common exceptions, exemptions, the validated CIS, the COE, and how to complete your annual report. Immunization Status Reporting

• All public and private schools, and licensed preschools must report the immunization status of students by Nov. 1 each year per state law (WAC 246-105-060).
• Schools can find reporting information on our school reporting web page. If you work in a preschool, preschool reporting information is also available online.
• Review step-by-step instructions to complete the school immunization report.
• Send all questions about immunization reporting and requirements to: oicpschools@doh.wa.gov.
• Report in one of three ways (more details below):
  
  • Use the WA State Immunization Information System (IIS) OR
  • Send in a Skyward or another student information system (SIS) report OR
  • Through the IIS School Module
  
  

Reporting through the WA State Immunization Information System (IIS)

• If you are new and will use the IIS to report, please send an email to the IIS Help Desk to get a reporting account: waiis@scientifictechnologiescorporation.zohosuppor t.com

• If you reported in the IIS last year and will use the IIS to report this year, you will receive an email with your username and password. If you don’t see this email, please let us know by sending an email to oicpschools@doh.wa.gov

• Do NOT use the IIS to report preschool data. Skyward users and other SIS users see information below. Preschools not using Skyward or other SIS must mail us the report. More information can be found here.

Reporting for schools using Skyward or another student information system (SIS)

• Create a Skyward export or electronic data file from your SIS
• The file must be in an Excel or .csv format (we cannot accept a .pdf)
• Email the file for schools AND preschools to oicpschools@doh.wa.gov

Reporting for Districts and Private Schools using the IIS School Module

• We will be contacting you with more information about pulling your report from the School Module. In the meantime:
  
  • Keep your Roster up to date
  • Add immunization dates using medically verified records
  • Enter exemptions and Chickenpox history
  
  
• Send questions about the School Module to: SchoolModule@doh.wa.gov
